Young Adult Community Navigator
About Project
The Young Adult Community Navigator is a FREE new service that helps young people aged 16-25 with their mental health and wellbeing. It is run by Barnardo's in Harrow.
What happens once a referral is made:
- The Community Navigator will arrange to meet the young person to learn more about them and understand their needs.
- Then they will work together to create a unique wellbeing plan for the young person based on their needs and changes the young person would like to see in their life.
- The Community Navigator can direct and refer young people to appropriate services, both local and national.
- The young person will get regular check-ins and support, tailored to them. Some examples include confidence-building exercises, teaching the young person techniques for managing low mood and anxiety, helping the young person to identify their strengths and how they can use them, and goal setting. It could even just be a weekly phone call to see how the young person is managing.
- Overall, the community navigator’s aim is to empower young people with the skills and resources to take control of their mental health and wellbeing.
How to access the service:
A young person aged 16-25 can refer themselves, or a local professional (e.g School Staff, GP) can refer a young person. Please download and fill in the Referral Form below and email it to harrowhorizons@barnardos.org.uk
